An inspection of prisons has found that there is no clear policy for dealing with the issues of overcrowding, drug use, and the reporting of deaths in prison.

According to the report of the Inspector of Prisons, which covers the period between March 2009 and September 2010, overcrowding is the biggest problem in the system.

Judge Michael Reilly, who inspected the prisons, also found that there are no standards against which inspections of prisons can be benchmarked.
