The family of a 53-year-old woman who died from breast cancer earlier this year say she was given the all clear on two separate occasions.

The family of a 53-year-old woman who died from breast cancer earlier this year say she was given the all clear on two separate occasions.

Two internal reports by the HSE, which have not been published, have found that serious errors were made in Ann Moriarty's case.

Her husband is calling for an independent review into the matter.

Ann Moriarty was admitted to Ennis General Hospital in June 2007 with a fever.

She underwent a chest X-Ray and was discharged four days later.

In August 2007 she was examined at the hospital again - this time she was discharged with a prescription for a stomach upset.

Days later she travelled to Galway for a second opinion and was informed that she had cancer and was terminally ill.

She died in April of this year.

Her husband, Karl Henry, has since received two unpublished HSE reports into his wife's treatment at Ennis - it reveals that the X-rays were misread and that highly abnormal blood tests which showed elevated tumour markers were not acted upon.

Mr Henry is calling for an independent investigation into the manner in which is wife's case was handled.

For its part, the HSE says it has met with Mr Henry on a number of occasions and is anxious to meet with him again.
