Twenty-nine-year-old Adrian Dunne, his 24-year-old wife Ciara and their two children, Leanne and Shania, were found dead at the family home on April 23rd, 2007.

Just days beforehand, the couple had contacted an undertaker to plan the family's funerals.

He passed his concerns to the authorities, but there was no out-of-hours social service to intervene in the case.

An official report earlier this week concluded that, even if gardaí or social workers had called to the Dunne house, the tragedy would probably not have been averted.

However, it did recommend than a national out-of-hours social service be established.

Several other recommendations were blacked out in the report, but the Government has denied engaging in a cover-up, saying the sections had to be censored for legal reasons.
