Gulf of Mexico spill caused by 'cascade of failures'

A leading US senator has blamed "a cascade of failures" for the Gulf of Mexico oil spill.

Jeff Bingaman likened the crisis to the sinking of the Titanic, as the three companies linked to the leak appeared before Congress.
