HSE audit raises concerns over vetting of non-EU consultants

The internal audit also found a lack of compliance with mandatory requirements in regards to both English-language proficiency and occupational health clearance.

A significant number of consultants recruited by the HSE from outside the EU may not have undergone Garda vetting or had their fitness to practise properly verified, according to a report.

A HSE internal audit found that governance arrangements for the recruitment of non-EU consultants were "unsatisfactory" and required "urgent" action to address shortcomings.

The audit considered the cases of 40 non-EU consultants recruited in the Dublin Midlands and Dublin North-East HSE regions in 2024, across seven separate hospital sites. 

They had never worked within the Irish health system previously.

It found that at least five of the recruits had no Garda vetting, identity verification, or references noted, while at least 10 had received no police clearance from an overseas force. 

They also lacked evidence of competency to work through English and were unable to provide occupational health clearance confirming they were fit to carry out their roles.

Auditors found low levels of compliance with mandatory requirements covering both English-language proficiency and occupational health clearance. They warned that this heightened patient safety risks because of the potential for clinical instructions to be misunderstood.

Just four of the 20 consultants reviewed in the Dublin-Midlands region were able to provide evidence of occupational health clearance.

In the same region, 14 of the 20 consultants had undergone Garda vetting, with 15 of the consultants having produced evidence of overseas police clearance — a mandatory requirement for all HSE employees who have worked abroad for a period of six months or more.

At least three of the overseas police clearances submitted to the audit were out of date.

Identity verification records were unavailable for six of the eight consultants examined at one location in the Dublin North-East region.

'Significant inconsistency'

The audit found "significant inconsistency" in how control testing was applied across the two regions.

Of the seven hospitals reviewed, only two — one in each region — achieved full compliance with English-language competency.

In terms of Garda vetting, one region achieved 100% compliance   — while in the other region, only one of three sites was found to be fully compliant.

The probe recommended that controls over international recruitment should be “strengthened” to ensure all mandatory pre-employment checks have been completed before the commencement of patient-facing duties, adding that the standardised checklist for such employment checks should be used across all sites.

The audit also recommended that the deficiencies identified among the consultants selected for review be examined immediately and that “any outstanding gaps should be remediated without delay”.

HSE management agreed to all recommendations stemming from the audit.

A spokesperson for the HSE said patient safety is its “foremost priority”.

They said: “The HSE has a comprehensive, standardised process for the recruitment of non-EEA doctors to ensure compliance with legal, regulatory, and professional requirements, and to verify that doctors are appropriately qualified and credentialed for their roles.

“The HSE is committed to ensuring that these requirements are consistently applied across all services and that the audit recommendations are fully implemented through established governance and oversight mechanisms."
