HSE outlines terms of inquiry into Tallaght Hospital

The Health Service Executive has published the terms of reference of the inquiry into the mishandling of X-rays and GP referral letters at Tallaght Hospital in Dublin.

It follows the recent revelations that almost 60,000 X-rays at the hospital were not read by a consultant radiologist - while thousands of GP referral letters went unopened.
