The North's Police Ombudsman has criticised the PSNI in relation to an incident two years ago in which a 28-year-old man was crushed by security gates at a police station in Co Tyrone.

Ciaran Connolly died of injuries to his head and upper body after being caught in the gates at Strabane Police Station in March 2003.

Ombudsman Nuala O'Loan said today that her investigation had found that the safety mechanism on the gate was not working properly and had not been fixed despite being reported a year earlier.

She said this amounted to a "corporate failure" on behalf of the PSNI as nobody took responsibility for ensuring the necessary repairs were carried out.
