Safety failures at all levels of the BP business were to blame for the Texas City refinery explosion which killed 15 people, a new report said today.

The US Chemical Safety Board’s final report, which comes three days before the second anniversary of the March 2005 disaster, said the explosion was brought about by “organisational and safety deficiencies” within the oil giant.
