ComReg has requested a report from O2 into the circumstances which led to the overcharging of 70,000 of its customers through billing errors.

In the report, ComReg want to know how such a situation arose, why internal safeguards failed to pick up these errors and what measures O2 is now taking to prevent any reoccurrence.
