Gardaí and HSE investigate death of elderly Cork woman
Gardaí and the Health Service Executive have both launched separate investigations into the death of a 65-year-old patient at a mental health facility in Cork last month.
The woman died on June 22nd at Heatherside Hospital near Buttevant, apparently after she slipped inside a secure chair and choked on her lap belt.
The Gardaí are investigating the case, while the HSE has also said it will be bringing in an outside team including a medical consultant and a director of nursing to carry out its own internal inquiry.
The agency says it has met with the patient's family and will continue to keep them informed as the investigation proceeds.
