Patient advocate groups say that they largely welcome the changes, but warned they need to be followed through on.

Mark Molloy - whose son died just 22 minutes after being born in Portlaoise Hospital - said he hoped the changes would prevent any repeat of what his family went through.

He said: "These are all the elements that unfortunately were happening before Mark died. There had been a number of incidents in Portlaoise and it wasn't pulled together to spot that there was potentially a problem there.

"Now that's all in place so that, fingers crossed, we don’t have to face anything like this again."
