Gaps in social services 'lead to death of child'

A report into the death of a teenager in State care is to reveal that there was a failure to recognise the risks faced by Danny Talbot.

The Child and Family Agency will today publish reports into deaths and serious incidents relating to children in State care.

At least one is expected to highlight serious gaps in social services.
