An independent examination of breast cancer services at Barrington's Hospital in Limerick has found that there was a "significant and avoidable" delay in diagnosing two cases.

The review, which examined 285 clinical files, also says that the level of care provided in 118 of these cases failed to meet acceptable standards, but no lasting harm was caused.
