A "once-off interpretive error by a locum consultant pathologist in Cork University Hospital (CUH)" has been blamed for the misdiagnosis of cancer sufferer Rebecca O’Malley.

Ms O'Malley was wrongly given the all-clear for breast cancer in 2005, only to find out the following year that she did have breast cancer, and that it had spread beyond the original tumour.
