Consultant ophthalmologist Michael O’Keeffe said management had kept on trying to cover up problems — and had kept on making one excuse after the other.

Prof O’Keeffe, based at the Mater and Temple St hospitals in Dublin, has been concerned for a long time that nobody is challenging the public system.
