The Health Service Executive is to carry out a critical incident review following the death of a newborn baby after he was transferred from Cavan General Hospital to the National Maternity Hospital, Holles St.

When the decision to deliver the baby by caesarean section was made, the main theatre at Cavan was in use and a second had to be prepared. The baby was born critically ill and later died at Holles St.

A multidisciplinary team will take part in the HSE review to examine the circumstances surrounding the birth last week and the death of the infant.

The HSE has expressed its condolences to the baby’s parents, saying it would “closely examine the circumstances of this case”.
