A widow whose husband killed himself and the couple’s two children has denounced the HSE for probing the tragedy without her knowledge or involvement, or seeking her views.

Una Butler, whose husband John killed himself and their children Zoe, 6, and Ella, 2, on Nov 16, 2010, in Ballycotton, Co Cork, only learned of the inquiry after calling for one herself.

The HSE document, written in response to the tragedy, cleared medics of missing any warning signs.

She revealed the situation after breaking her public silence for the first time since the deaths.

She spoke out to highlight alleged mental health law gaps — including the lack of risk checks to protect children from potentially dangerous parents.

Speaking via a statement by solicitors Ernest Cantillon and Sinead O’Carroll, she said: “During the course of meetings with Kathleen Lynch, the minister informed Una that the HSE carried out an investigation.

“This came as a complete surprise as Una was wholly unaware of any investigation. If she had been aware, she would have participated,” the statement read. “The report, carried out by the HSE into its own actions, concludes there were no shortcomings. Una strongly disagrees.

“No one could legitimately deny her the right to be involved in any inquiry into Ella and Zoe’s death. Any investigation should be carried out by an impartial body, not the body being investigated.”

At 1pm yesterday, the Irish Examiner sent the HSE questions asking why Ms Butler was not told of the probe, and why her views were not sought. The inquiry’s scope, length, interviewees, and findings were requested. The HSE South was asked if it was normal for next-of-kin not to be told of probes or have their views sought, and whether Ms Butler’s opinion will now be taken into account.

In a response at 9pm, sent after repeated calls, no questions were answered. A spokesperson said the HSE “is in ongoing contact with Ms Butler and committed to addressing her concerns”.

The spokesperson said it did not carry out a formal inquiry at the time of the tragedy as Mr Butler had previously been discharged. His discharge plan was sent to those overseeing his care.
