A WOMAN who took her life in the psychiatric ward was not on suicide watch by hospital staff, despite being told of two suicide attempts by her the same day.

An independent investigation was last night demanded into the circumstances surrounding the suicide of Anne Carroll, who slashed her wrists and stabbed herself hours prior to being found hanging in the psychiatric ward at Cork University Hospital.

Dan Neville TD, president of the Irish Suicidology Association said Ms Carroll, 37, should have been under 24-hour suicide watch given what had happened earlier in the day.
