THE Health Service Executive (HSE) has described the failure of a nursing home to tell State authorities about the death of two of its patients as “unacceptable”.

The two patients, who died last year, were in a home in the Leinster area and due to the nursing home’s inaction, were buried without death certificates being issued.

Medical officials weren’t informed of one death until five months afterwards while they were never told about the second.
