DISASTERS like the explosion of the Deepwater Horizon rig could happen again without significant reform, according to the conclusions of a presidential panel that has the companies involved in the nation’s largest offshore oil spill pointing fingers at each other again.

BP has blamed itself, other companies’ workers and a complex series of failures for the massive Gulf of Mexico oil spill and the drilling rig explosion that preceded it.

BP engineers began pumping heavy drilling mud into the blown-out Gulf of Mexico oil well in what they believe is their best yet chance to snuff out for good one of the world’s largest spills.
