Weak leadership, misuse of resources and lack of co-ordination between services were the main reasons a strategic city in northern Afghanistan briefly fell to the Taliban, an investigation has found

International medical charity Doctors Without Borders has withdrawn from the northern Afghan city of Kunduz after a deadly air strike destroyed its hospital.

The international charity Doctors Without Borders said at least 16 people including nine local staffers were killed when its clinic came under "sustained bombing" in the northern Afghan city of Kunduz.

Military reinforcements have been sent to Kunduz, where government forces managed to fend off a major Taliban assault earlier this year.

Taliban fighters have captured more than half of a major northern city after launching coordinated attacks, Afghan police say.
