A report into a missed diagnosis of breast cancer for a woman at University Hospital Galway has said the diagnosis was missed on two occasions in the case of the woman, known as Ms A.

The breast-cancer screening centre at the Midlands Hospital in Mullingar is to close down, as part of a move towards a centralised system of cancer care services.

The Health Information and Quality Authority has announced plans to conduct a review of all Irish breast cancer services.
