It will use the survey findings, where applicable, to represent the views and desires of its suppliers with relevant policymakers at the Department of Agriculture and Food and in the European Union.
It says it is hoped the responses can help to develop and tailor supports and services for the benefit of all.
The questionnaire deals with all the principal concerns that are influencing supplier intentions.
Glanbia farm development manager John Fitzgerald urged all suppliers to return the questionnaire as soon as possible in a Freepost envelope.
