The Metropolitan Police has apologised to the family of Mark Duggan, whose shooting triggered last summer’s riots in the UK, for failing to inform them of his death.

The apology was revealed after Britain's Independent Police Complaints Authority upheld a complaint by the family that neither the Met or the IPCC formally notified them.
