An inquiry into the mistaken bombing of Canadian troops in Afghanistan has blamed two US F-16 pilots for failing to exercise proper caution.

Marine Lieutenant General Michael DeLong, deputy commander of US Central Command, said the cause of the deadly accident was "the failure of the two pilots to exercise appropriate flight discipline."
