O'Sullivan let off hook in report
The IRFU has been presented with the findings of the official review into Ireland's dismal World Cup campaign.
The report by the Genesis consultants group found that management, players and the union collectively underperformed.
The assessment says top players did not take part in enough high-level matches in the run-up to the tournament and thus lacked the required level of match practice.
In order to address the problems outlined in the report, the IRFU has undertaken to make a further appointment to the management team - someone who will play an as-yet undefined supporting role to Eddie O'Sullivan.
The union has also pledged to appoint a dedicated backs coach and to retain a professional who will provide psychological support to players and management.
