A mother whose seven-year-old daughter died at the Children’s Hospital Temple Street has said she believes the hospital’s “lack of staffing and proper processes” failed them on the day.

Olivea Maguire also warned those caring for loved ones in hospital to remain vigilant and to keep asking questions.
