The HSE has failed to show its final report into the death of Savita Halappanavar to her widower — despite promises it would be handed over before going to the Cabinet.

Praveen Halappanavar’s solicitor Gerard O’Donnell said the fact ministers have seen the report before Savita’s family is the latest error on the part of officials handling the case.

Speaking to the Irish Examiner last night, he said that after the report was leaked in February, Mr Halappanavar decided he wanted to take part in the inquiry.
