‘Vital’ chances to intervene were missed

This case dealt with a young man who was in care from the ages of six to 18, and who was found dead following an accident while sleeping rough.

According to the report, while there was no direct link between the incident and the action of the services, “vital opportunities” were missed to intervene in the life of a young man of “considerable potential, yet very vulnerable”.
