According to the report, while there was no direct link between the incident and the action of the services, “vital opportunities” were missed to intervene in the life of a young man of “considerable potential, yet very vulnerable”.

Taken into care on the grounds of parental neglect, Q suffered a traumatic early childhood characterised by paternal domestic violence, alcohol abuse and neglect.
