DETAILED concerns over the safety of pre-birth care in Ireland were raised in a confidential report four years ago.

As figures show one woman is contacting the HSE’s emergency helplines over the crisis every five minutes they are open, it has emerged that issues central to the scandal were noted in a 2006 Trinity College document.

The report, recommendations from which are only now being acted on, was drawn up after the first national survey of all obstetrics services in Ireland.
