Any patient with a suspected delayed breast cancer diagnosis should trigger a formal incident response and senior management should be notified. The patient should be informed of the findings and outcome as a priority.

The HSE should urgently review communications processes, policies and procedures that hospitals use to respond to patients when there is a serious incident.
