A YOUNG Co Clare student died after being struck by a train because of “systematic and avoidable” failures, the Old Bailey in London heard yesterday.

Michael Mungovan, aged 22, from Quilty, Co Clare, was doing temporary work on the lines at Vauxhall, south London, when he was struck from behind by an empty train.

The Brunel University student had only worked on the railway a few times before and was standing in for a friend.
