Berlusconi gets to keep the home, said to be worth €78m, the Corriere della Sera newspaper said.
The paperwork was filed on Christmas Day at Milan’s tribunal. It was not clear how long the terms lasted.
Ms Lario announced she was divorcing the billionaire media mogul in 2009, citing his presence at a the 18th birthday party of a Naples girl and his fondness for younger women.
The couple met in a dressing room in 1980 after Berlusconi saw Ms Lario perform in a Milan theatre, were married in 1990 and have three grown children. He has two children from his first marriage.
The ruling said the money would allow Ms Lario to “live in the manner that she was accustomed to during her marriage” and it also noted she was “without a home as part of the settlement”.
The 76-year-old politician is engaged to a woman about 50 years his junior.
He is currently on trial accused of having underage sex with a prostitute and has also thrown his hat into the ring for the general election which takes place in February.
